The Source: Planning Your Last Days

Your plan for your last days is the most difficult and likely the most important conversation you haven't had yet. Waiting till it is too late has happened to too many people argues Dr. Angelo Volandes in his book "The Conversation: A Revolutionary Plan for End-of-Life Care." 

Volandes makes the point that patients need to empower themselves to make these plans and communicate them because "End of life care in America is broken on every level." It extends life for no other reason than because it can, and causes far more pain and unneccessary burden for families and for patients.
